When a defect or problem occurs, human nature naturally tends to blame the most obvious and easily accessible "cause": the operator made a mistake, the machine stopped, the part broke. However, these are not the origins of the problem; they are merely the symptoms surfacing. Root Cause Analysis (RCA) is a philosophical questioning technique that rejects these superficial excuses, aiming to unearth the mechanical or systemic origin of the failure.
The "5 Whys" is the simplest yet most lethal weapon of this analysis. Much like a curious child asking incessant questions, the answer to every "Why?" becomes the premise for the next "Why?". Typically, by the fifth question, a fundamental systemic gap (the root cause) is revealed—such as the machine failing because "the lubrication manual was missing" or "procurement bought the wrong grade of oil." Any action taken (a band-aid) before reaching the root cause does nothing but guarantee that the exact same error will confront you again another day.
