The greatest fallacy in occupational health and safety architecture is attempting to stop risk solely through personal protective equipment (PPE). The hierarchy of controls is a structural algorithm descending from the most effective method to the weakest intervention. At the pinnacle, "Elimination" completely removes the hazard. If the source cannot be destroyed, "Substitution" steps in to replace it with a less harmful system.

If the problem originates from machinery or space, "Engineering Controls" establish physical barriers, preventing human encounter with error. While "Administrative Controls" shorten working hours and introduce training, the lowest level, PPE, is the final desperate line to rely on when all other barriers collapse. A robust system chokes the problem at its source and leaves no room for human error.
