The Fault Line Beneath the Surface Crack

In companies, problems are exactly like weeds. If you only cut a weed at ground level (treat the symptom), it will grow back stronger a few days later. For the weed to be completely eliminated, you must get your hands dirty in the soil and pull it out by its roots.

Root Cause Analysis (RCA), situated at the heart of ISO standards and Lean Manufacturing, is an analytical approach that rescues managers from the illusion of the "Quick Fix" and forces them to dig deep into the problem. Picture 1

The 5 Whys Technique

When a problem occurs, traditional companies ask only one question: "Who did it?" Root Cause Analysis focuses on the question "Why did it happen?" and asks this question 5 times in a row (5 Whys).

Let's say a customer complained that a product was defective:

  1. Why did we send a defective product? Because the machine on the production line cut the part incorrectly. (Symptom)
  2. Why did the machine cut the part incorrectly? Because the machine's calibration was off. (Apparent Cause)
  3. Why was the calibration off? Because the monthly periodic maintenance was not performed. (Process Failure)
  4. Why was the periodic maintenance not performed? Because there is a shortage of maintenance technicians; the current team is overloaded. (Resource Failure)
  5. Why is there a shortage of maintenance technicians? Because Human Resources stopped hiring new staff 6 months ago due to a budget cut. (Root Cause)

If you had stopped at step 2 (just recalibrated the machine), the problem would seem solved, but it would fail again 1 month later. The 5 Whys analysis proves with a mathematical chain that the true root cause of a simple production error on the floor is the "Board of Directors' Budget Decision."

Corrective Action

Once the root cause is found, Corrective Action comes into play. Corrective action is not repairing the defective product (That is called Correction). Corrective action is making the systemic change that ensures that error NEVER happens again.

(For example, adding software that automatically locks the machine from operating if maintenance has not been performed).

The strength of a management system (ISO 9001 Clause 10.2) is measured by how well it learns from its past mistakes. Root cause analysis is the mechanism that transforms errors (wastes) into sources of knowledge (Lessons Learned) that increase the system's intelligence.
